STEP ONE: Download and fill out the application and waiver.
IMPORTANT - PLEASE READ
If you are submitting your payment through PayPal, you MUST also send an email
with your child's intent to participate, or submit paperwork if you will be new to our
system.  PayPal will not notify us of your child's participation which may result in them
not being placed on the participant list.

STEP TWO: US Lacrosse Numbers ARE MANDATORY, no exceptions.  Double check that you have put both your USL number and
date of expiration
.  If you do not have a US Lacrosse number, go to www.uslacrosse.org to obtain one PRIOR to the start of the SLC
event.
STEP THREE: Attach your payment to the application and waiver and send them to P.O. Box 162882, Sacramento, CA 95816, or bring
them to the field on the first day of the clinic or league.  If you pay via PayPal, please note it on the application form.
IMPORTANT INFORMATION REGARDING PAYPAL
Select the date of the event and enter into the secure PayPal link.  
Please be sure to note the player's name with the payment, especially if you do not share the same last name.

In the days following your submission, you will receive a confirmation email.  Should you have any questions or concerns in the mean time,
feel free to email us and we will respond as quickly as possible.
