| STEP TWO: US Lacrosse Numbers ARE MANDATORY, no exceptions. Double check that you have put both your USL number and date of expiration. If you do not have a US Lacrosse number, go to www.uslacrosse.org to obtain one PRIOR to the start of the SLC event. |
| STEP THREE: Attach your payment to the application and waiver and send them to P.O. Box 162882, Sacramento, CA 95816, or bring them to the field on the first day of the clinic or league. If you pay via PayPal, please note it on the application form. |
